POLICY FOR PRESCRIPTION REFILLS

If you are on daily medication we do require you to have scheduled follow up visits. If you need a refill before your office visit you may contact your pharmacy and they will fax us a refill request. Refill requests may take up to 48 hours. Please be aware we do not refill medications when the office is closed.

CO-PAYS AND BALANCES

All co-pays and balances are due at the time of your office visit.

As a courtesy to our patients we will submit charges to your health insurance companies. However, you as the patient are ultimately responsible for payment on any services provided by our office.

APPOINTMENTS

We confirm all scheduled appointments 2 days prior to your office visit.

NO SHOW POLICY

If you need to cancel an appointment we require a 24-hour notice to avoid a potential fee of $40.00. If you need to cancel a prime appointment (7am-9am or 5pm-7pm) we require a 48-hour notice. In order to offer our patients same day sick appointments as well as convenient times for scheduled visits this policy is strictly enforced.

FORMS

If you need a form filled out by your provider there will be a fee of $25.00. Some forms may be higher depending on the complexity of the form. Forms are usually ready in 2 weeks.

WORKERS COMPENSATION

We will treat established patients who have been injured at work. Before we may see you the following information is required: Name, address, phone and fax numbers of Worker’s Compensation Carrier. Contact person’s name, phone number and extension, the claim number and the date of injury. If another provider besides a walk in or emergency room has treated you, you will need to obtain permission from your worker’s compensation carrier before being seen at APCM.
