  Associates in Primary Care Medicine is proud to have achieved the status of a Patient Centered Medical Home

           As your medical home, we are working harder than ever to provide the most comprehensive and collaborative care possible.  Especially for patients with a chronic illness, we are focusing on a more team-oriented approach to your care.  We are very excited to welcome Karen Sciamacco, RN, BS, CCM, CDOE to our care team.  She is a highly qualified educator and care coordinator.  No doubt, you will benefit from this more intensive approach to your care.
