Developmental Screenings:
Due to recent recommendations by the American Academy of Pediatrics to include more formal developmental assessments in babies and toddles, we have started to use several validated screening tools.
One such tool is the PEDS-DM or PEDS: Developmental Milestones measurements for Children 0-8 years. The PEDS-DM assesses each developmental domain: gross motor, fine motor, expressive language, receptive language, self-help, and social-emotional development. Screening is done at the 9, 18, and 24-month well child visits and other times as warranted.
The M-CHAT is another screening tool used between the ages of 16 and 30 months of age to assess for autism-spectrum disorders. This questionnaire can be downloaded and completed prior to the 18 and 24-month well child visit. This can be reviewed and scored at the visit.
How to Prepare for your child's well visit exam:
If your child is 18 months or 24 months please complete the M-CHAT form and bring it with you to the appointment
If your child is age 11 or over please review the Gardasil Information Sheet
For all well-child exams, please complete the well-child questionaire and bring to the exam
Review the Car Safety/Prevention Information
If you have any physical forms that need to be filled out, please complete your portion of the form
